With the (suspicious) death of its previous long-lived Sultan, Vilhun has underwent a long-since done election of the noble families.
The Free Lord families have been mostly out of the picture, having bureaucratic experience but ultimately having no eligible candidates. The Horde Lord families had elected the khan of the Suli Clan, which was then immediately rebuked by Steel Lord families emir of the Hagane Clan.
The clash has since then caused a governmental paralysis and with no leverage from other houses, expanded into an all out war between the two clans, both residing on the Isle of Flod. Other clans of those two bloodlines, while not having declared war on each other, have become hostile and broadly distanced themselves from each other, at least until the affair is over.

The second in command of the previous Sultan has since become regent of Vilhun. Unfortunately, that second-in-command is a sovereign dragon. While she has been fiercely royal to the sultan, this has inherently attracted unwanted attention from the rest of the world towards Vilhun. Mogxa especially, who deeply remember the age of the Draconic Empire.
With the war (and the election process itself) having divine connotations, it has also attracted attention of the Crusaders and by extension, the Sanitorium.
